This applies to Chaves County
All persons summoned for jury duty in the Fifth Judicial District Court in Chaves County must complete the Juror Qualification and Juror Questionnaire Forms.
Once the Juror Qualification and Juror Questionnaire Forms have been filled out, save them, attach them to an e-mail and send to firstname.lastname@example.org
If you are 75 years of age or older:
- You may be permanently excused from jury duty by completing an Affidavit of Age available from the jury clerk at the courthouse or online by clicking on the form below. Please complete the form and return it to the District Court Clerk’s office with your paperwork.
If you are not a resident of Chaves County:
- You must complete the Non-Resident of County or State affidavit. This form is available to print by clicking the form below. This form must be notarized and a copy attached of: current driver’s license, current voter’s registration, or current utility bill with the prospective juror’s full name listed along with the Juror Qualification Form.
If you are not a citizen of the United States:
- You must provide proof which must include potential juror’s name and address. Acceptable forms of documentation: Current VISA, passport, green card, permanent resident cards or other legal documentation that clearly states not a US citizen.
All persons summoned for jury duty in the Fifth Judicial District Court, who have not been excused, must participate in jury duty.
